Crossroads provides residential services that are tailored to meet each individual’s needs in a variety of settings. Typically, all services are funded through the State of Alaska’s Home and Community Based Medicaid Waivers as Residential Habilitation.
One on one support provided throughout the community for adults, who live in their own homes. These supports can be provided anywhere from a few hours a week to 24 hours per day depending on and individual’s assessed needs.
Benefits To Choosing A Supported Living Environment:
Learning new skills
Choice of Direct Service Professionals
Group Home Support: On-site supports provided 24 hours per day in homes maintained by Crossroads and licensed as Assisted Living Homes by the State of Alaska. These services are tailored to provide additional support for adults who require a more intensive level in order to build the skills to live in a supported living environment.
Crossroads offers one-to-one Day Habilitation services. As with all of Crossroads’ services, Day Habilitation is individualized to meet each service participant’s needs. These services focus on teaching new skills or maintaining/improving existing skills in self-help, socialization, or adaptive skills while in the community.
CCATS Day Habilitation is provided in a non-residential setting, separate from the participant's private resident or another residential setting, to an individual participant. Day Hab promotes the development of skills needed for independence, autonomy, and full integration into the community.
Our counseling services are built on the therapeutic relationship between you and your counselor. Our therapists approach focuses on the person as a whole not the individual parts and pieces. We provide a safe, private and relaxed environment to support you through your therapeutic goals.
The role of Counselor is supportive rather than directive and at Crossroads we focus on Person-Centered supports that are tailored to your specific needs.
